Cryostorage Transfer Consent Form
The University of Colorado Advanced Reproductive Medicine (CU ARM) program will be closing on June 6, 2021.
Patients who have frozen specimens stored with CU Advanced Reproductive Medicine will need to complete the cryostorage transfer consent form below by March 23, 2021.
Note: If you are patient of Dr. Santoro being seen for complex menopause management, please call 720-674-7157.

Cryostorage transfer options on the consent form below include:
- Ship to new Shady Grove Fertility (SGF) lab: See more details here.
- Ship to CryoGam long-term storage: CU ARM has entered an agreement with CryoGam, CO for free transportation to their long-term storage facility. CryoGam is the only commercial cryobank in Colorado, serving patients since 1990. They offer personal cryopreservation, directed or known donor processing, fertility testing of sperm, sperm preps for IUIs, anonymous donor sperm, and long term storage of sperm, oocytes, and embryos. - Ship to another facility of your choosing.
- Thaw and discard.
All patients with cryostorage, complete the following specimen transfer consent form by March 23, 2021:
